Facility Request Form

Please read the information below and then fill out the request form.

Thank you for your interest in using our church facilities. Please complete this form in full to request use of any church space for your event or activity. Once submitted, your request will be reviewed by our staff during the next Monday staff meeting. After discussion, a designated staff member will contact you to inform you whether your request has been approved or if additional information is required.
